I have discussed in my previous post that the Philippine Regulation Commission (PRC) came to Laoag City to facilitate the filing of application for the Nursing Licensure Examination (NLE) or Nursing Board Exam on November 2008. Results of this exam will be posted here as soon as possible so stay updated. This time around, I will present the required documents for the Nursing Licensure Examination or Nursing Board Exam as many know it.

The following requirements are to be submitted by the applicants (personal appearance needed, proxy not allowed) to the application officer or PRC official/employee:

• Birth Certificate (NSO and/or Local Birth Registration)
• Marriage contract, if married
• Residence Certificate
• Transcript of Records
• Summary of Related Learning Experience
• Operating room and Delivery room cases with notarization
• Identification photo, 3 pieces

Examination fee amounting to PhP 900 is also to be paid upon filing of application. After paying the examination fee, the applicant’s requirements, application form and official receipt from PRC are submitted to the chief of application division for approval.

Upon approval, a Notice of Admission signed by the Chief of Application Division is given to the applicant along with a copy of the following: Program of the Philippine Nurse Licensure Examination, Examinee’s Self-Instruction Sheet and Examinee’s Guide.
